What causes hip pain in active adults?

Hip pain in active adults usually comes from restricted joint mobility, muscle imbalances around the hip and pelvis, or both working together to create compensation patterns that load the wrong structures under movement. The pain itself can show up in the front of the hip, deep in the glute, down the outside of the leg, or even in the lower back, depending on which pattern is driving it. Identifying the source accurately is what determines whether treatment actually works.

Why Hip Pain Is Frequently Misdiagnosed

Most active adults with hip pain have been told to stretch their hip flexors, foam roll their IT band, or strengthen their glutes. Sometimes that helps temporarily. Usually it does not stick. The reason is that the actual driver of most hip pain is joint mobility restriction, not tissue tightness.

When one hip is missing internal or external rotation, your body finds a way to get that movement from somewhere else. Usually the lower back. That compensation pattern puts chronic stress on structures that were never designed to handle it, which is why so many hip pain patients also have back pain and cannot figure out why.

The IT band and TFL tightness follows the same logic. The TFL overworks because the lateral hip stabilizers are not doing their job efficiently. Rolling out the IT band relieves the tension for a day. Restoring lateral hip stability changes the pattern.

The clicking in the hip during exercise is often a sign of exactly this kind of imbalance. A structure snapping over a bony prominence because the joint is not centered and moving the way it should. It is worth assessing, not ignoring.
